A useful distinction
A suggestion made during a meeting is not automatically an adopted decision. A question can remain unresolved even when the conversation moves on. Clear records prevent later readers from mistaking enthusiasm or silence for a completed agreement.
Put it into practice
Use short entries labelled decision, action and open question. Note the date and applicable approval process. Share the draft through the group’s normal review procedure, keeping private details limited to those who need them. Do not invent exact quotations from memory.
